GUIDELINES for enrollment
IMPORTANT
PLEASE READ THOROUGHLY UP TO THE VERY END
ATTENTION : INCOMING FRESHMEN / MASTERAL / DOCTORAL STUDENTS or ENROLLEES
INCOMING QUALIFIERS FOR THE PROGRAM SCHEDULE OF MEDICAL EXAMINATION WILL BE FROM MONDAYS TO FRIDAYS ONLY.
I. FIRST PHASE: LABORATORY TESTS COMPLIANCE
1. Secure the LABORATORY REQUEST FORM from the University Clinic only.
2. Present laboratory request form to any diagnostic clinic/medical facility of your choice.
3. Take note on the ff. VALIDITIES:
CBC – 1 week only
URINALYSIS – 1 week only
CHEST X-ray – 1 week only
DRUG TEST – 1 year only
II. SECOND PHASE: ONLINE SUBMISSION & APPOINTMENT SCHEDULING
1. Once with LABORATORY RESULTS, upload on the link below and choose your appointment date & time for physical and dental examination at the University clinic (CNU CLINIC).
Note: Multiple appointments will not be catered and automatically deleted.
2. Upload LABORATORY RESULTS ONLY. No envelopes, receipts and X-ray Films.
III. THIRD PHASE: PHYSICAL and DENTAL EXAMINATION (Releasing of Medical Certificate)
Bring the ff. on THE DAY OF APPOINTMENT:
3.1. Duly accomplished MEDICAL EXAMINATION REPORT
***Download the form thru PDF FILE and print in A4 SIZE bondpaper. (DO NOT CHANGE SCALE BELOW 100% WHEN PRINTING)
***Kindly fill-out the ff:
I. DEMOGRAPHIC PROFILE
II. INCASE OF EMERGENCY
III. HEALTH/MEDICAL HISTORY
IV. PHYSICAL EXAMINATION (Height, Weight, LMP(females only) )
*** Use BLACK/BLUE ballpen only.
3.2 Laboratory Results (as reference in case File Uploaded is blurred or portal is down).

IMPORTANT REMINDERS:
- Verification of documents (laboratory results, dental and medical examination and releasing of medical certificate) will be done STRITCLY BY APPOINTMENT.
Note: Availability of slots are in the link above. - Medical Examination Report Should be in A4 SIZE bondpaper.
- Distorted and crampled documents will NOT BE ACCEPTED.
- BE ON TIME on your appointment time. Be present 30 MINUTES before your appointment time.
LATE COMERS WILL NOT BE ENTERTAINED.
