NO ENVELOPES, RECEIPTS or FILMS

Kindly get the laboratory request form from the University Clinic only.

INCOMING QUALIFIERS FOR THE PROGRAM SCHEDULE OF MEDICAL EXAMINATION WILL BE FROM MONDAYS TO FRIDAYS ONLY.

***mAKE SURE TO HAVE successfully UPLOADED ALL LAB. RESULTS @ THE SAID LINK PROVIDED

BRING THE FOLLOWING on the day of YOUR APPOINTMENT:

1. Duly accomplished MEDICAL EXAMINATION REPORT
(print in A4 size bondpaper)
kindly fill-out the ff:

I. DEMOGRAPHIC PROFILE
II. INCASE OF EMERGENCY
III. HEALTH/MEDICAL HISTORY
IV. PHYSICAL EXAMINATION (Height, Weight, LMP(females only) )

PLEASE DO NOT CHANGE THE SCALE BELOW 100% WHEN PRINTING
***MEDICAL EXAMINATION FORM SHOULD BE FILL- OUT ALREADY , use BLUE or BLACK BALLPEN ONLY

IMPORTANT REMINDERS:

Verification of uploaded lab results, dental and medical examination and releasing of medical certificate will be done STRITCLY BY APPOINTMENT ONLY. Note: Appointment slots/ availability are in the link above.
Required documents should be in A4 SIZE bondpaper.
Distorted and crampled documents will NOT BE ACCEPTED.
