IMPORTANT
PLEASE READ THOROUGHLY UP TO THE VERY END
ALL INCOMING FRESHMEN/ TRANSFEREES/ GRADUATE and DOCTORAL STUDENTS
UPLOAD the following on this link:
- CBC (1 week validity only)
- URINALYSIS ( 1 week validity only)
- CHEST X- RAY (6 months only)
***scheduling of appointment is applicable to ALL INCOMING FRESHMEN/ TRANSFEREES/ RETURNEES and 1st semester GRADUATE STUDY STUDENTS ONLY!!!
*MID SUMMER GRADUATE STUDY STUDENTS MAY COME AS WALK-IN FOLLOWING THE CLINIC OPERATING HOURS .
BRING THE FOLLOWING on the day of YOUR APPOINTMENT:
1. Duly accomplished MEDICAL EXAMINATION REPORT
***Download the form thru PDF FILE or MS OFFICE WORD FILE and print in a SHORT SIZE bondpaper
- DEMOGRAPHIC PROFILE
- HEALTH HISTORY
- PHYSICAL EXAMINATION ( HEIGHT and WEIGHT ONLY of GENERAL DATA)

3.Laboratory Results Laboratory Results (as reference in case uploaded files either blurred or unreadible)
IMPORTANT REMINDERS:
- Submission of documents as mentioned above will be done by APPOINTMENT only.
- Required documents should be in SHORT SIZE bondpaper.
- Distorted and crampled documents will NOT BE ACCEPTED.
- BE ON TIME on your appointment time.
*** Please be present 30 minutes before your appointment time.
