Guide for Online and Face-to-face Application
Applicants who are from Cebu are encouraged to apply physically. Applicants outside Cebu may opt for online application.
Incomplete requirements will not be entertained.
APPLICATION GUIDE
Application period:
Main Campus – November 8, 2023 – January 24, 2024 for AY 2024-2025
Please take note of the following:
- An undergraduate applicant can take an admission/entrance test, only once whether in Main Campus or any External Campuses. Violation of such instruction is a forfeiture of all his/her admission/entrance test results.
- Those who are outside Cebu may opt for online application.
- Acceptance of applications is from Monday to Friday until 4:00 PM only. (except holidays)

FACE TO FACE APPLICATION
Requirements:
- Certified true copy of Senior High School Report Card, showing a GPA of 85, LRN, and Senior High Track and Strand (Grade 12 – 1st quarter or 2nd quarter grading)
- Certificate of Good Moral Character (Original)
- Photocopy of PSA issued Birth Certificate (present Orig.)
- 3 pcs. 2×2 colored ID picture with name tag
- 1 pc long brown envelope with plastic cover
- 1 pc long folder with plastic cover

Application for Doctor of Medicine Program Academic Year 2024-2025
Application period: January 29 – May 15, 2024
Applicant Qualifications:
Applicants should possess the following qualification prior to taking the CNU College Admission Test:
- A natural-born or a naturalized Filipino citizen;
- With outstanding scholastic record from a Bachelor’s Degree (BS) in a field relevant to Health from a college or university duly recognized by the CHEd;
- Had taken the National Medical Admission Test (NMAT) and at least in the 60% percentile rank or better. Validity of NMAT result is within two (2) years from the time of admission;
- Must have good moral standing as certified by the university/college graduated from in the BS degree;
- Priority will be given to applicants with low family income; and
- Preferably from Central Visayas (Region 7).

- 2×2 colored picture with name and signature (Scanned or Picture Copy of the original)
- Transcript of Records (Scanned or Picture Copy of the original) – Average rating of 2.0 of all subjects in the Bachelor’s degree and Certification of GWA
- Accomplished College of Medicine Testing Admission Form attached 2×2 colored picture with name and signature (download and print pdf form, then, fill out and scan) (pdf downloadable form)
- Valid Identification Card (Scanned or Picture Copy of front and back)
- NSO/ PSA Certificate of Birth (Scanned or Picture Copy of the original)
- Letter of Recommendation from either of these two references: (Scanned or Picture Copy of the original)
- Academic Head(s) of the school graduated
- Professional/ Work station head/Professor
9. Good moral certificate (Scanned or Picture Copy of the original)
10. ITR of earning family members or BIR Certification of Exemption (Scanned or Picture Copy of the original)

Please note that online applications will only be from 8:00 AM – 5:00 PM (Mondays-Fridays except Holidays). We can also entertain at most 35 applicants per day.
Schedule of Interview
- All applicants will be interviewed by the CNU-VSMMC CM Admissions Committee;
- The Admission Committee submits the interview scores to the Dean, who will endorse the scores to the Testing Center
- Testing Center integrates the scores in the computation of the final ratings

Admission 2nd-Semester (Start of Class: January 2024)
September 29, 2023 – November 24, 2023 including Diploma in Professional Education (2nd sem)
Class Period: January – June 2024
1. The applicant should prepare the following documents to be uploaded:
a. Graduate Studies Applicant Profile Sheet (from the College) (pdf downloadable form)
b. Scanned or Picture Copy of 2×2 colored picture with name and signature
c. Transcript of Records (scanned or picture copy)
d. GS Testing Admission Form with 2×2 colored picture with name and signature pasted/embedded on the form (pdf downloadable form)
2. Applicant will accomplish the Google form of the application for entrance examination:

3. College evaluates the TOR.
If the applicant’s bachelor degree will not qualify for the master’s program applied or applicant’s master’s degree will not qualify for doctorate program, the College/Department will send an email that the application is declined (end of transaction).
4. College Dean or authorized representative signs the GS Testing Admission Form for the successful applicants as to academic requirement and submits it to the Testing Center for verification of their Philippine residency.

5. Testing Center submits to accounting office the signed GS Testing Admission Form.
6. The accounting office will email the applicants as to order of payment and the link of the accounting google form for the applicants to be accomplished. The accounting office will cc the Testing Center on the “For Payment GS/DPE Testing” email thread. (Testing Center can view the accounting link for those who have paid the Testing fee).
7. Accounting office validates the payment and when there are no more required documents from the applicant then accounting will send the applicant’s control number to the “Payment GS/DPE Testing” email thread. Having acquired the control number. Applicant can proceed to step number 8 below.

8. Applicant should prepare the following documents for uploading:
a. GS Testing Admission form with signature of the College Dean b. Valid and recent Identification Card with picture and signature
9. Applicant will accomplish the Graduate Studies Admission Testing Form 2

10. Applicant will receive an email from the Testing Center (schedule of face to face examination and test guidelines) within 2-3 days after filling out the Testing Center’s Google Form 2.

Age Requirement: The applicant must be 2-2.5 years old for Toddlers, 3-3.5 years old for Nursery 1 and 4-4.5 years old for Nursery 2 as of October 31, 2024.
