College of Nursing

The College of Nursing was created through CSC Board Resolution No. 35 s. 1982. Through this board, Cebu Normal University entered into a consortium with Southern lslands Hospital (now known as Vicente Sotto Memorial Medical Center). Since then, the college has offered Bachelor of Science in Nursing with only one section for each level. lt had its first graduates in 1986.

Online Admission Requirements

Online application will start on October 29 – December 10, 2021 for AY 2022-2023. 

 
Requirements for the Online Application and Testing for College Applicants 
1. Senior High School Report Card or Certified true copy of it showing a GPA of 85, LRN, and Senior High Track and Strand (scanned or picture copy)
2. Certificate of Good Moral Character (scanned or picture copy)
3. PSA issued Birth Certificate (scanned or picture copy)
4. Scanned or picture copy of a valid Identification Card (front and back images)
5. Accomplished College Online Applicant Profile Sheet (for perusal of the Colleges as to the needed information of the UniFAST) (pdf downloadable form)
6. Accomplished College Online Testing Admission Form (pdf downloadable form)
7. One (1) selfie close-up picture (recent and decent) for the College Online Testing Admission Form
8. 2×2 colored picture with name and signature (scanned or picture copy)
9. Personal email address
10. Zoom application (for testing)
11. Reliable internet connection, at least 3mbps (during testing)
12. Laptop/computer set with web camera (during testing)

CNU-CN Request of Scholastic Records Form - Google Form link

Note: This form is required for all College of Nursing clients who would like to request scholastic records. The College shall forward the document to the University registrar for dry-seal and payment processes at registrar@cnu.edu.ph. The client shall be duly notified by the College on the progress of the request through the email address reflected in this form. Please prepare verification document as needed like scanned TOR. The applicant should prepare the following documents to be uploaded:
a. Graduate Studies Applicant Profile Sheet (from the College) (pdf downloadable form)
b. Scanned or Picture Copy of 2×2 colored picture with name and signature
c. Transcript of Records (scanned or picture copy)
d. GS Testing Admission Form with 2×2 colored picture with name and signature pasted/embedded on the form (pdf downloadable form)
