KINDERGARTEN

  1. PSA issued Birth Certificate (scanned or picture copy)
  2. Accomplished Kindergarten Testing Admission Form (pdf downloadable form)
  3. 1 copy of 2×2 colored picture with name (scanned or picture copy)
  4. Personal email address of the parent or guardian
  5. Testing fee of P300

SECONDARY

  2. Report Card with GPA of 85 or above (irrespective of quarter, depends on what is obtainable)
  3. Certificate of Good Moral Character (scanned or picture copy)
  4. Accomplished Secondary Testing Admission Form (pdf downloadable form)
  5. 2×2 colored picture with name (scanned or picture copy)
  6. Scanned or picture copy of one (1) valid ID
  7. Testing fee of P300

SECONDARY

  1. Report Card with a GPA of 85 or above (irrespective of quarter, depends on what is obtainable)

  2. Photocopy of PSA issued Birth Certificate (present Original)
  3. Valid ID
