1. Upload laboratory results (CBC, CHEST X-RAY, URINALYSIS and DRUG TEST) on the link of the previous page as highlighted in blue.

2. Choose and follow your appointment date and time as instructed.

3. Strictly follow the instructions in there most especially on what documents to be uploaded.

4. DO NOT PRINT THE MEDICAL EXAMINATION REPORT. YOU ALREADY HAVE A FILE IN THE CLINIC.
